Working with our Integrated Care Partnerships and Primary Care Networks

Primary Care Networks [PCNs] are evolving and shaping to meet local configuration and need. Across NENC region, PCNs are in different places of evolution. However, it is clear that they are the future vehicles for integrate, place-based collaboration and delivery. The NE&NC Ageing Well network has 4 ICP Ageing Well leads that sit on our regional ICS steering group, representing their PCNs. Although PCNs are evolving and have local priorities, Ageing Well is core to all. So, the network is actively focusing it works through a PCN ‘lens’ – supporting, piloting and evaluation. We aim to strengthen the connection with PCN leadership, as we embed the national Ageing Well priorities in Primary Care and Community Services.

Integrated Care Partnerships. The NENC ICS has 4 Integrated Care Partnerships (ICP) which support place-based delivery of their ageing well ambitions. Each ICP has an ‘Ageing Well’ lead who is a member of the Ageing Well Steering Group and represents their ICP within in the Community of Practice with other ICP and specialist expert colleagues. The regional Ageing Well is supporting ‘place-based’ ageing well delivery through access to our Community of Practice, involvement in regional and place-based projects and offers access to measurement, research and evaluation via NECS and the regional ARC multiple Morbidity and Frailty work stream. In 2020/21, the regional Ageing Well steering group established a ‘supportive’ approach to oversee National Ageing Well Service Development Funding (SDF) allocation to ICPs. Through the NENC Ageing Well network and governance arrangements, we are currently further building our approach.

In 2021, the Regional Ageing Well Steering Group established a ‘supported and critical friend’ approach to oversee national ageing well funding allocation. We are currently building on this approach through our Community of Practice for long term support to ICPs and place-based areas in their ageing well journeys.

In 2021, the following ICP proposals for ‘ageing well’ funding were submitted and supported by the ageing well steering group and NHS England:
