An Example of a Frailty Pathway

The following is an example of how we can see our Frailty I-Care Toolkit translated into a ‘pathway’ of care. Clearly, each local health and care economy (LHE) would have an approach that is locally unique in delivery. It is crucially important that each LHE starts to ‘map out’ local delivery underpinned by consideration of the workforce, skills and training implications.
