Introduction

The Care Closer to Home Programme for Cumbria, the North East and Hambleton, Richmondshire and Whitby (part of the region’s shadow Integrated Care System) has prioritised frailty as an area of focus.  Working in partnership with the Academic Health Science Network for the North East and North Cumbria, a frailty toolkit – ‘Frailty I-Care’ – has been developed, reflecting the components of effective care ‘Involve, Consider, Assess, Respond and Evaluate’.

The toolkit aims to support the development of better ways of preventing frailty and supporting people, carers, families and communities living with frailty.  It will be used to support regional ambitions for frailty and to enable each local health and care economy [LHE] to implement best practice solutions and achieve optimum outcomes for their local population.
